Fall Registration ends Saturday, July 31st.

Procedures for All Players:
1. Log onto http://www.eayso.org.

2. Apply for your child as a player and print out 3 copies of the forms. Make a copy of proof of age for your child if they are new to our region (birth certificate, passport, Baptismal certificate, etc.)

3. Apply for both parents as volunteers (required for the protection of all children, even if you do not plan to volunteer) & print out 2 copies of the forms. Make a copy of each parent’s Driver’s License.

4. Sign all forms.

5. Prepare a check for $155/player made out to AYSO Region 795.

6. Mail all forms (player registration, volunteer registration, new player proof of birth, Driver’s License, & payment) to the following address:
