NEW: Fall Registration begins April 1st.

Procedures for New Players:
1. Log onto http://www.eayso.org (not before April 1st).
2. Apply for your child as a player and print out 3 copies of the forms.
3. Apply for both parents as volunteers (required for the protection of all children, even if you do not volunteer).
Sign all forms.
4. Make a copy of proof of age for your child (birth certificate, passport, baptismal certificate, etc.)
5. Make a copy of your Driver’s License.
6. Bring all forms and copies to one of our registration events (see below). Mandatory new player evaluation will be held at our registration events.
7. If you cannot attend the registration event, please mail all forms to the following address:

Procedures for Existing Players:
Preprinted forms will be available at our registration event.
If you cannot attend the registration event, please print the existing form for your child at www.eayso.org. Sign and mail in 3 copies.

Fees
$140/1 child, $260/2 children, $360/3 children, $440/4 children
Spring participants: $126/1 child, $235/2 children, $325/3 children, $400/ 4 children
After June 30th, fees increase to $155/child, no sibling discount.
No refunds issued after June 30th.

May 22nd 10 am – 2 pm Lawrence Middle School
New player evaluation times:
10-11: 4-6 year olds
11-12: 7-9 year olds
12-1: 10-11 year olds
1-2: 12 & up

June 12th 12 pm – 4 pm
New player evaluation times:
12-1: 4-6 year olds
1-2: 7-9 year olds
2-3: 10-11 year olds
3-4: 12 & up
Schedule:
Opening Day is scheduled for Saturday, September 11th.
